Information I Collect

When you book an appointment or visit, I collect personal information such as your name, contact details, medical history, and treatment notes either in person or online via the Jotform platform. This is necessary to provide you with safe and effective massage therapy.

 

How I Collect Information

I collect your information in the following ways:

  • Online: Via an online form using Jotform.

  • In-Person: Through paper forms if necessary.

How I Store and Protect Your Information

I take the security of your information seriously:

  • Paper Forms: All paper records are stored in a locked filing cabinet at 82 Ilchester Crescent

 

  • Digital Information: All digital data is stored on a password-protected laptop. Digital forms from Jotform are imported into a client database on Fresha. Details are stored on my booking platform Fresha in order for them to determine which bookings are returning clients and which clients they have found. I ensure both platforms are password-protected.

 

 

How I Use Your Information

I use your information to:

  • Provide you with professional and safe massage therapy.

  • Contact you regarding your appointments.

  • Request feedback on my services.

  • Send you marketing information, but only if you have given your explicit consent by ticking the consent box.

  • Keep a client database on my private account on Fresha to recognise which clients are new clients and which are existing as I pay a fee on new client bookings via Fresha

Sharing Your Information

I will never share your personal information with third parties without your explicit consent, unless required to do so by law.

 

Your Rights

You have the right to consent to receive marketing information. If you have opted-in, you can opt-out at any time by contacting me directly.
